1. Open Photoshop
2. Take New Document (see here - How to Open New Document in Photoshop )
   
   
   
   
3. Insert any Image ( see here - How to Insert Image in Photoshop )
   
   
   
   
4. Select Layer for which you need to apply Layer Mask and Click on Add Layer Mask Option from Layer Pallet
   
   
   
5. A White color box will be displayed beside Image Layer called Mask
   
   
   
6. Select the Mask (white color box)
7. Choose Gradient Option from Toolbar
   
   
   
8. Select gradient as black & white from Gradient editor (located at property bar)
   
   
9. Apply Gradient
10. Done
